The otoscope: a look inside the ear

The first tool is usually the simplest. An otoscope is a handheld light with a magnifier that lets the audiologist look down your ear canal to the eardrum. In a few seconds it can reveal a wax blockage, signs of infection, or fluid sitting behind the drum, any of which changes how the rest of the visit goes. If wax is in the way, we may pause for earwax removal before testing, since a clogged canal can skew the results. The exam is painless, and nothing goes deeper than the outer canal.

The audiometer and pure-tone testing

The audiometer is the device most closely associated with a hearing test. Wearing headphones or small earphones in a quiet booth, you listen for a series of tones at different pitches and respond each time you hear one. By finding the softest level you can detect at each pitch, the audiologist maps your hearing on an audiogram, the chart that shows where your hearing is sharp and where it has slipped. This is the backbone of a hearing evaluation, and it takes only a few minutes per ear.

Tympanometry and the middle ear

Pure-tone testing shows what you hear; tympanometry shows how your middle ear is working. A soft probe in the ear canal changes the air pressure slightly and measures how your eardrum responds. It does not test hearing directly. Instead it flags problems like fluid behind the drum, a perforation, or a stiff or overly mobile eardrum, each of which affects how sound passes through. The probe feels like a light pressure change, similar to the pop you notice on a plane, and the reading takes only a moment.
